Tools to help individuals assess their substance-related risk.

Purpose

These self-assessments are offered with the intention of empowering individuals to gauge their risk of experiencing substance-related difficulties.  If you have questions about them or experience technical difficulties, please reach out to our office.

Limitations

The below screening tools are not intended to serve as a substitute for a formal substance use evaluation. If you feel that it would be beneficial for you to receive substance-related care, please speak with a healthcare provider.
